GLP-1 medications such as Ozempic, Wegovy, Zepbound, and Mounjaro have changed the landscape of obesity treatment. For many individuals who have struggled for years with excess body weight, these medications are producing results that once seemed impossible. People are losing significant amounts of weight, improving blood sugar control, lowering cardiovascular risk factors, and often regaining a sense of hope that had been missing after countless failed attempts at dieting. That’s the good news. The less-discussed news is that weight loss and improved physical health are not always the same thing. Over the past year, researchers, physicians, exercise physiologists, and fitness professionals have begun asking an increasingly important question. While GLP-1 medications are undeniably effective at helping people lose weight, what happens to muscle mass, strength, physical function, and activity levels during that process? The answer may have important implications for anyone currently taking these medications or considering them in the future.
A Not-So-Surprising Discovery About Physical Activity
Sadly, I called this one a while ago. You can call me pessimistic, but I call it premonition based on my experience working with people for 25 years in this space. Conventional wisdom would suggest that losing weight makes movement easier. A person carrying fifty fewer pounds should theoretically find walking more comfortable. Climbing stairs should require less effort. Exercise should feel less intimidating. As body weight decreases, it seems reasonable to assume that physical activity would naturally increase. Here is the frustrating part: Researchers presenting findings at ENDO 2026 (the Endocrine Society’s Annual Meeting) expected to see something along those lines when they examined activity data from adults with obesity who had begun GLP-1 therapy. Instead, they found the opposite. Average daily step counts declined from just over 5,000 steps per day to fewer than 4,500 steps per day after participants started treatment. Moderate-to-vigorous physical activity also decreased significantly. Despite successful weight loss, participants generally became less active rather than more active. At first glance, that may seem like an interesting but relatively minor observation. In reality, it may be one of the most important findings emerging from the current wave of GLP-1 research. Because exercise is not simply a method of burning calories. Exercise is one of the primary signals that tells the body what tissue should be preserved.
Why Weight Loss Is More Complicated Than Most People Realize
One of the biggest misconceptions in the health and fitness industry is the idea that all weight loss is automatically beneficial. Most people step on a scale, see a lower number, and assume everything is moving in the right direction. The reality is more nuanced. The scale only tells you how much weight was lost. It doesn’t tell you what was lost. When body weight decreases, that reduction can come from several sources. Some of it may be body fat. Some may be water. Some may be glycogen. Unfortunately, some may also come from lean tissue, including skeletal muscle. This isn’t unique to GLP-1 medications. It occurs during virtually every form of weight loss intervention ever studied. Whether weight loss occurs through traditional dieting, bariatric surgery, medication, or a combination of approaches, lean tissue loss often accompanies fat loss to some degree. The question isn’t whether some lean mass will be lost. The question is whether we’re doing everything possible to minimize it.
Muscle Is More Than Appearance
When most people hear the word muscle, they immediately think about aesthetics. They picture bodybuilders, fitness influencers, or individuals pursuing a specific physique goal. That perspective misses the bigger picture. Muscles play a critical role in nearly every aspect of physical function. It helps us climb stairs, carry groceries, maintain balance, recover from injuries, regulate blood sugar, and preserve independence as we age. Muscle is also closely tied to metabolic health and long-term quality of life. This becomes increasingly important after the age of forty. As we grow older, building muscle becomes more difficult while losing muscle becomes easier. Recovery slows. Hormonal changes occur. Physical activity often decreases due to career demands, family responsibilities, or previous injuries. For this reason, preserving lean tissue during weight loss may be just as important as losing body fat in the first place. A smaller body is not necessarily a healthier body if significant amounts of strength and muscle are sacrificed along the way.
What the Research Is Beginning to Show
One of the more encouraging developments in recent research is that exercise appears to dramatically improve outcomes during GLP-1 treatment. What is so frustrating about this is that people are choosing to remain sedentary and ignore their fitness while on these drugs at alarming rates. This makes no sense, considering a recent review examining GLP-1 receptor agonists and exercise found that combining structured exercise with medication produces superior results compared to medication alone. Researchers observed improvements in body composition, metabolic health, inflammation, and long-term weight-loss maintenance when exercise was included as part of the intervention. The evidence suggests that exercise and GLP-1 therapy are not competing approaches. They are complementary ones. This distinction matters. Too often, conversations about GLP-1 medications become polarized. Some individuals view them as miracle drugs that eliminate the need for lifestyle changes. Others dismiss them entirely and insist that exercise alone should be the answer. The research increasingly suggests that both positions miss the mark. Medication may help create the opportunity for meaningful weight loss. Exercise helps determine the quality of that weight loss.
The Goal Should Never Be Weight Loss Alone
One of the phrases I frequently use with coaching clients is that weight loss is rarely the actual goal. People don’t truly want to weigh less. They want what weighing less represents. They want to move without pain. They want more energy. They want better health markers. They want to keep up with their children or grandchildren. They want confidence. They want freedom. The scale is simply one measurement that may reflect progress toward those outcomes. Unfortunately, when weight loss becomes the sole focus, people can lose sight of the bigger objective. If someone loses fifty pounds but becomes weaker, less active, and less physically capable in the process, we should question whether that outcome fully achieved its purpose. A successful transformation is not measured solely by pounds lost. It is measured by health gained.
What GLP-1 Patients Should Focus On
For individuals currently using GLP-1 medications, the takeaway is not fear. The takeaway is strategy. The medication can help reduce appetite and facilitate weight loss. That is precisely what it was designed to do. However, preserving muscle and maintaining physical function requires additional effort. Resistance training provides a signal that muscle tissue is still needed. Adequate protein intake supplies the raw materials required to maintain that tissue. Daily movement supports overall health, energy expenditure, and cardiovascular fitness. Proper hydration and recovery help ensure the body can adapt effectively throughout the process. None of these habits are new. What’s changing is our understanding of how important they may be for people experiencing rapid weight loss through medication-assisted approaches.
The Future of Obesity Treatment
The future of obesity treatment will likely focus on more than simply reducing body weight. Researchers are already exploring ways to improve body composition outcomes by preserving lean tissue while maximizing fat loss. As the science evolves, success may become less about how much weight a person loses and more about what percentage of that weight comes from fat versus muscle. Frankly, that’s a shift that should have happened years ago. For too long, the scale has dominated conversations about health and fitness. Yet anyone who has spent time interpreting DEXA scans, InBody reports, or body composition assessments understands that the number on the scale tells only a small part of the story. The real goal has never been to become lighter. The goal is to become healthier, stronger, more capable, and more resilient. GLP-1 medications can absolutely help people move toward that goal. But they work best when paired with the habits that support long-term health. The medication may initiate the journey. Movement, strength training, proper nutrition, and consistency are what help ensure you arrive at the destination in the strongest possible condition.
Bottom line: Sustainable habits, proper nutrition, and consistent exercise will always beat out a pharmaceutical in the end. Drugs dont fix the behaviour that got you in the situation that caused you to start taking them in the first place. Believe that.
